Senior Community Older Persons Mental Health Practitioner

While we ideally seek a Registered Mental Health Nurse (RMN), we also welcome applications from experienced Social Workers with a strong background in mental health and dementia care.

  • 5 days a week Mon – Fri 9am – 5pm, Havant and Waterlooville CMHT OPMH Substantive Based in Havant Health Centre
  • 5 days a week Mon – Fri 9am – 5pm, Havant and Waterlooville CMHT OPMH 1 year Fixed Term – Based in Havant Health Centre
  • 3 days a week Petersfield and Bordon CMHT OPMH Substantive Based in Chase Hospital

About the Role:

In this position, you will provide expert mental health care to older adults within the community. Responsibilities include providing specialist mental health assessments and interventions for older adults both organic and functional.

Work collaboratively with service users, carers, and other professionals to develop and implement care plans.

Support safeguarding processes and risk management.

Contribute to multidisciplinary team discussions and service development.

On a rota basis you will act as a duty triaging nurse within the team base.

Our teams are collaborative and supportive team members openly help each other, and value transparency committed to delivering high-quality person-centred care for older adults.

Main duties of the job

Key responsibilities:

  • Conducting thorough memory assessments for older adults
  • Performing initial mental health assessments and risk evaluations
  • Managing duty referrals and urgent mental health concerns
  • Coordinating and reviewing care plans within the CPA framework
  • Supporting patients in developing strategies to maintain independence
  • Collaborating with multidisciplinary teams to deliver holistic care
  • Providing advice and support to families and carers
  • Provide supervision and mentorship to B5 nursing staff.

What We\’re Looking For:

  • Registered Mental Health Nurse (RMN)
  • For Social Workers: Registration with Social Work England and demonstrable experience in mental health and dementia care.
  • Experience working with older adults and mental health conditions
  • Knowledge of memory assessments and CPA processes
  • Strong communication and assessment skills
  • Ability to work flexibly and respond to urgent referrals

  • Registered Mental Health Nurse or Social Worker.
  • Recognised Mentorship qualification (applies to nurses) or preparedness to undertake this within agreed timeframe
