We are expanding our award-winning Urgent Community Response and Hospital at Home Virtual Ward service across Mid and North Hampshire, and we are looking for experienced Consultant Practitioners to join us. This is a pivotal role for experienced advanced clinical professionals with a strong background in frailty. You might have developed your expertise in Hospital at Home services, acute frailty care, emergency settings or community urgent response. Whatever your path, you will bring both clinical credibility and leadership experience, along with a record of driving service improvement and managing complex care with autonomy and confidence.

While the role is offered at Band 8C, we also welcome interest from candidates who meet most of the criteria and would benefit from structured development through our Foundation Pathway. Hampshire and the Isle of Wight is one of the most advanced areas in the country for Consultant Practitioner roles. Our Trust has a high number of multi-professional Consultant Practitioners, supported by a strong advanced practice strategy and a collaborative, well-connected peer network. You will not be isolated in your role; you will work alongside, and be supported by, colleagues who understand the consultant level landscape and are actively shaping its future.

This is a high impact role aligned with the NHS England Multi-Professional Consultant-Level Practice Capability and Impact Framework. You will demonstrate expert clinical decision-making in complex cases, contribute to strategic service development, support learning and development across the system, and champion a culture of research and innovation, all while delivering truly person-centred care.

Although this role is aimed primarily at physical health professionals, we also welcome applications from experienced mental health practitioners. Many of the patients we support are living with co-morbidities or secondary mental health needs, and your insight could make a real difference to their outcomes.

Essential Qualifications and Experience:

  • Professional UK registration e.g. NMC / HCPC with evidence of sufficient experience to meet the requirements of the Job Description. Open to individuals with background in Nursing or AHP.
  • Advanced Clinical Practice Master's degree OR a health related master's degree AND evidence of equivalence of Advanced Level practice capabilities via the portfolio route.
  • Independent non-medical prescribing or pharmacology level 7 profession relevant qualification.
  • Evidence of working at academic level 8 and or through competency-based assessment of application of level 8 working.
