Overview
An opportunity has arisen for an Associate Practitioner within our Complex Health Team (CHT) in Portsmouth\’s Integrated Learning Disability Service – ILDS hosted by Portsmouth City Council. The service has been rated as Outstanding in the last 2 CQC inspections.
Role purpose
As an Associate Practitioner you will work as part of the CHT in providing high standards of effective health care for people accessing outreach, enhanced supportive living services and or in receipt of Continuing Healthcare funding. Your role will be varied, working with service users, family, and carers to establish and maintain positive professional relationships.
Responsibilities
- Provide high quality, efficient and effective specialist healthcare for adults with Learning Disabilities with complex health needs, in partnership with other agencies and carers.
- Deliver person-centred care that is comprehensive and responsive to the needs of individuals and their family carers.
- Assess identified needs, implement treatments and evaluate outcomes in collaboration with service users and family carers.
- Work under the direction and supervision of a registered practitioner.
- Act as a named worker within the CHT to lead and support an inter-disciplinary approach to care in supported living services and outreach service users living in the community.
- Coordinate care, develop and deliver support plans and annual reviews; monitor and review packages of care as appropriate.
- Monitor and support complex healthcare packages responsive to changing health needs (e.g., challenging behaviour, mental health needs, and complex physical health).
- Promote the philosophy of care in line with organisational values; advocate on behalf of individuals and support self-advocacy where appropriate.
- Plan and coordinate healthcare packages responsive to changing health needs, including specialist assessments and interventions.
- Support the development of health action plans in line with Valuing People recommendations.
