Clinical Partner – what\’s involved in the role?
Manage the Clinical Care of clients in the Western Sydney Region for HammondCare at Home. This is a Full-time position across Monday to Friday 9.00-5.00pm
- This position is ideal for a Registered Nurse seeking more stability and work-life balance – no shift or night work!
- Responsible for clinical assessment of each Home Care Package client, ongoing clinical support/advice, and hands-on clinical care as required
- Support with COVID escalations and care worker education
- The Clinical Care Manager leads, supports, and mentors their team
- The role involves professional accountability and responsibility aligned with Registered Nurse registration requirements, managing and providing direct care for clients’ clinical needs
What do you need to apply?
- Registered Nurse with a current AHPRA registration
- At least 2 years of experience as a Registered Nurse, with wound management experience (skin tears, pressure injuries, complex wounds)
- Strong knowledge of Home Care packages, preferably with experience managing a team across multiple sites
- Willingness to travel between sites, hold a current Australian driver’s license, and have access to a personal car
