Job overview
The post holder will work with the wider multi-disciplinary team to manage and co-ordinate the provision of diabetes nursing care to the renal inpatient and outpatient population.
You will be responsible for leading a specialised service, providing nursing input as a clinical expert to support and facilitate high quality, individualised care.
You will be a resource for patients and the clinical team, acting a professional role model providing direct and indirect care.
Main duties of the job
- Provision of specialist, high quality, expert nursing care and support for renal patients (and their carers) with diabetes in liaison with the multidisciplinary team(s) and other health agencies.
- To act as a resource for patients and the clinical team by providing comprehensive advice, information, teaching and training with regards to diabetes in kidney disease both within ICHT and externally.
- To undertake, where appropriate, nursing research with published outcomes, and updating own knowledge to promote excellence in clinical practice, and the setting of quality standards within speciality.
- Contribute in audit processes. Formulate an annual report/clinical audit of own CNS service and outcomes.
