Hamilton Woods Associates are currently recruiting for a Supported Housing Team Leader to provide keywork support and deliver individual and group based interventions focused on personal development and successful resettlement into the community.

Duties and Responsibilities of the Supported Housing Team Leader:

  • Complete client inductions, pre-placement meetings and visits to prisons or other locations, ensuring clients understand the service, expectations, rules and requirements.
  • Provide keywork support and deliver individual and group-based interventions focused on personal development, independence and successful resettlement within the community.
  • Support the assessment and management of risk, monitoring any changes in behaviour, risk of harm or reoffending and sharing relevant information through appropriate procedures.
  • Maintain clear and accurate case records, risk management plans and other documentation, making appropriate referrals and working closely with probation services, police, external providers and partner agencies. xiskglj

Essential Requirements of the Supported Housing Team Leader:

  • Enhanced DBS
  • Full UK Driving Licence
