Our Team: Oakmeadow is a 29 bedded unit, this includes 19 ICMB beds for period of assessment short term before returning home. Oakmeadow have 10 transitional beds, these are used for short term placement. Your role will be to deliver care and support to service users to enable them to meet their goals with the personal care plans and support them in this process. You will help with any personal care needs and encourage independence.

Hours: 1x 26.29, 1x 21.66 hours per week

Your Contributions:

  • NVQ Level 2 or willingness to work towards
  • Previous Experience in providing person-centred care in line with individual care plans
  • Skilled in maintaining verbal and written records and effectively communicating with service users and colleagues
  • Supporting, prompting, assisting and administering medication; utilising equipment i.e. Hoists, stand aids and free ways
