Do you have a passion for changing the lives of children and young people? We are currently looking for two part-time Outreach Workers to join our Care Leavers Team!
- X1 Permanent post - 18.5hrs
- X1 2 year FTC - 30hrs

Our Team: Our Care Leavers Team is a friendly and supportive team that is passionate about helping young people make a successful transition from care into adulthood. We work alongside care-experienced young people aged 16 to 25, providing practical and emotional support to help them achieve their goals and become as independent as possible. We support young people with all aspects of adult life, including accommodation, education, employment, training, finances, health and wellbeing. We understand that every young person's journey is different and aim to build positive, trusting relationships that enable them to feel supported, listened to and empowered. As a team, we are committed to being aspirational for the young people we work with, celebrating their achievements and supporting them through challenges. We work closely with partner agencies and local services to ensure care leavers receive the support they need to achieve the best possible outcomes and reach their full potential. Your Contributions: As an Outreach Worker, you will assist in the provision of services to enhance the ability for all young people leaving care and so enable them to realise their full potential and become successful in all aspects of their lives.
- Providing person-centered support to help young people develop independent living skills and integrate into their communities.
- Delivering housing support, including tenancy-ready courses, preventative work, and interventions.
- Facilitating the care leaver drop-in and hub, ensuring access to essential services and support.
- Building strong relationships with the young people, advocating for their rights, wellbeing, and best interests.
- Maintaining records and working collaboratively with Personal Advisors, partner agencies, and other professionals to ensure effective service delivery.
