The Transitions Team consists of Social Workers and Community Care Workers. The team works with young people from the age of 14 to 25 who have a learning or physical disability, supporting them to transition from Children’s Services to Adult Social Care. The team applies its Care Act 2014 duties and responsibilities and works with a number of services including Statutory, Voluntary and the Private Sector, building relationships for better outcomes for the people we support, ensuring that the Preparing for Adulthood agenda is embedded in your approach.

Our Team:

As an Advanced Social Worker within the Transitions Team, you will support the operational implementation of Strengths and Rights Based approaches within Adult Social Care. You will also support in the development of the teams approach, including embedding the Mental Capacity Act 2005 into every day Social Care Practice. You will assist in the supervision of qualified and non qualified staff.

As well as providing professional advice, mentoring and supporting your colleagues, you will provide information, advice and awareness to provider services, stakeholders and professionals, helping to ensure that the service we offer in health and social care is consistent, legally defensible and helps achieve the best outcomes we can for the people we support, and their families and carers.

We offer 26 days’ annual leave, rising incrementally to 34 days after 20 years’ service. We support flexible working and offer a generous pension. You will enjoy excellent support and supervision, with regular opportunities to discuss and reflect on your practice, both with your line manager and peers.

You must be a qualified Social Worker who is registered to practice with Social Work England, demonstrating a commitment to ongoing professional development. You must be a qualified Best Interests Assessor and or Approved Mental Health professional, a qualified Practice Educator and progressed to an experienced Social Worker.

In Halton, the post will provide you with the opportunity to develop and enhance your skills in Social Work. And be part of the development of partnership working with health colleagues to enhance the experience of adults who use adult social care services. You must have experience of completing Section 42 Safeguarding Enquiries, Court work, completing Risk Assessments as well as experience of working closely with a range of services, stakeholders and professionals. Having knowledge and experience of working in Children’s Social Care is also desirable. In addition, understanding the Preparing for Adulthood agenda will be key in your role.
