About the Care Assistant role: Supporting Recovery at Home
We’re proud to support individuals transitioning from hospital to home through our Discharge to Assess (D2A) model. This approach allows people to return home as soon as they are medically stable, where their care needs can be assessed in a familiar environment.
As part of our team, you’ll play a vital role in:
- Providing short-term support to individuals recently discharged from hospital.
- Helping assess their ongoing care needs in the comfort of their own home.
- Supporting a process that reduces hospital stays and avoids unnecessary long-term placements.
- Collaborating with health and social care professionals to determine the most appropriate next steps, whether that’s reablement, continued care, or full independence.

The ideal Care Assistant is passionate about helping others and committed to delivering high-quality care and support. While previous experience in care, hospitality, retail, or customer service is welcomed, it’s not essential—we value the right attitude and a willingness to learn.
