Based in Leeds, this office‑based role involves responding to emergency telephone calls from service users, their families, Care Managers, and Care Workers. The coordinator provides support with queries, records incidents, and liaises with out‑of‑hours GP services, ambulances, local authorities and safeguarding teams.
Shift pattern: Weekends only – every weekend or alternate weekends, 6:00–14:30 or 14:30–22:15. Flexibility to cover holiday or sickness absences on the Monday‑Friday morning (6:00‑9:00am) and evening (5:00‑10:15pm) shifts would be an advantage.
Key Responsibilities
- Answer and triage emergency telephone calls.
- Log all incoming calls and document actions taken.
- Ensure all calls, incidents and sickness are accurately recorded and escalated if required.
- Assign care to workers using the electronic care planning system.
- Add medication to the electronic MAR charts and update changes to planned care in real time.
- Deal with out‑of‑hours GP services, ambulances, local authorities and safeguarding when necessary.
- Provide support and information to users, families and Care Managers.
- Maintain accurate records and documentation throughout the shift.

- Previous care experience is required.
- Must be able to work weekends and flexible shifts as described.
- Subject to an enhanced DBS check and reference verification.
