Hales Homecare is looking for compassionate and dedicated Hospital Discharge Support Workers to join our team in Gateshead. In this role, you’ll help individuals transition from hospital care back to living independently in their own homes.
What you’ll do:
- Provide hands-on care and support tailored to individual needs.
- Dynamically assess and respond to changing care requirements.
- Liaise with healthcare professionals to ensure continuity of care.
- Signpost service users and families to additional services, equipment, and resources for long-term recovery.
