Out of Hours Coordinator Based in Leeds. Shift Pattern Weekends only: every weekend or alternate weekends. 6:00–14:30 or 14:30–22:15. Flexibility to cover holiday and sickness absence on the Monday‑Friday morning shift (6:00–9:00am) and evening shift (5:00pm–10:15pm) would be an advantage.
Responsibilities
- Answer emergency telephone calls from service users, families, Care Managers, and Care Workers.
- Support callers with their queries.
- Document and report all calls, incidents and sickness; escalated when required.
- Log all incoming calls and actions taken.
- Assign care workers via the electronic care planning system.
- Add medication to service users’ electronic MAR charts and ensure updates are reflected in real time.
- Deal with out‑of‑hours GP, ambulances, local authorities and safeguarding when required.
- Participate in full induction training.
Qualifications & Requirements
- Previous care experience is required.
- Previous experience of working within a care or health environment is essential.
- Enhanced DBS check and reference verification required.
- Practical knowledge of electronic care planning systems is preferred.
