Home Care Assistant – Home to Assess
Location: Canewdon, Essex. Pay: £13.20 per hour plus mileage at £0.30 per mile. Hours: 3pm-11pm. Must be available 7 days a week including alternate weekends. A full UK driving licence and access to your own vehicle are required.
About the Care Assistant role
Supporting individuals transitioning from hospital to home through our Discharge to Assess (D2A) model. You will play a vital role in: Providing short‑term support to individuals recently discharged from hospital. Helping assess their ongoing care needs in the comfort of their own home. Supporting a process that reduces hospital stays and avoids unnecessary long‑term placements. Collaborating with health and social care professionals to determine the most appropriate next steps.
Responsibilities

Qualifications
