Home Care Assistant – Home to Assess
Based in Brentwood
Pay Rate: £13.20 per hour + mileage at £0.30 per mile
Hours: 3pm-11pm
Must be available to work across 7 days, including alternate weekends
A full UK driving licence and access to your own vehicle is required.

About the Care Assistant role: Supporting Recovery at Home
We’re proud to support individuals transitioning from hospital to home through our Discharge to Assess (D2A) model. This approach allows people to return home as soon as they are medically stable, where their care needs can be assessed in a familiar environment.
Responsibilities
- Providing short-term support to individuals recently discharged from hospital.
- Helping assess their ongoing care needs in the comfort of their own home.
- Supporting a process that reduces hospital stays and avoids unnecessary long-term placements.
- Collaborating with health and social care professionals to determine the most appropriate next steps, whether that’s reablement, continued care, or full independence.
What we’re looking for
- Passionate about helping others and committed to delivering high-quality care and support.
- Reliable and honest team player with strong communication skills, both verbal and written, to ensure accurate record-keeping.
- Previous experience in care, hospitality, retail, or customer service is welcomed, but not essential.
- Right attitude, willingness to learn, and shared values: a commitment to positive change, trust, professional excellence, and delivering personalised care.
- Right to work in the UK. (applications from Tier 2 sponsoring candidates currently not accepted.)
