Shift pay: £13 per hour
Guaranteed block hours - 11pm - 7am
Previous experience in providing care (essential)
A full UK driving licence and access to your own vehicle (essential)
Hales Homecare is looking for compassionate and dedicated Hospital Discharge Support Workers to join our team in Gateshead. In this role, you’ll help individuals transition from hospital care back to living independently in their own homes.
What you’ll do:
- Provide hands-on care and support tailored to individual needs.
- Dynamically assess and respond to changing care requirements.
- Liaise with healthcare professionals to ensure continuity of care.
- Signpost service users and families to additional services, equipment, and resources for long-term recovery.
The ideal Support Worker is passionate about helping others and committed to delivering high-quality care and support. You’ll be a reliable and honest team player with strong communication skills, both verbal and written, to ensure accurate record-keeping.
