Clinical Pharmacist in Cradley Heath

Main duties of the job:

• Demonstrate motivation and passion to deliver excellent service within general practice.
• Provide face‑to‑face structured medication reviews for patients with polypharmacy, multimorbidity, care‑home residents, and house‑bound patients, including prescribing changes as an independent prescriber and arranging necessary monitoring tests.
• Implement and lead medication optimisation and safety improvement projects, including developing prescribing policies, repeat‑prescribing re‑authorisation processes, and ensuring compliance with CQC standards.
• Support and guide long‑term condition clinics, including independent prescribing for chronic diseases (e.g., stable angina, anticoagulation management).
• Carry out pharmacy services at the retrieval points, including discharge reconciliation, transfer of care medicine management, and ensure continuity of medication supplies for high‑risk groups.
• Use computer searches and risk stratification tools to identify patients at risk of medication‑related harm, work with case managers and multidisciplinary teams to implement interventions to reduce readmissions.
• Design and implement population and public health campaigns, including specialist immunisation knowledge, within the network.
• Develop and run repeat‑prescribing policies, manage re‑authorisation, and coordinate with patients and primary care team to adjust medicines as needed.
• Provide clinical leadership on quality improvement: audit prescribing safety, conduct improvement projects, present findings, and support implementation of tools such as the PINCER tool.
• Represent and liaise with the PCN, GP practices, ICB Pharmacists, Clinical Leads, and other NHS/private organisations to promote consistent prescribing practices.
• Participate in training and education of primary health team members, visiting medical and nursing students, and pharmacy technicians on therapeutics and medicines optimisation.
• Engage in research and evaluation activities, including clinical governance, evidence gap identification, and local/national guideline implementation.
• Maintain professional activities such as continuous professional development, health and safety compliance, data protection adherence, and equality of care.

Qualifications:

• Higher degree in pharmacy and registration as an independent prescriber with the GPhC.
• Completion of a national CPPE training pathway for independent prescribing (preferred).
• Minimum of 2 years experience in general practice or a PCN, and evidence of at least 5 years postgraduate pharmacy experience.
• Strong written and verbal communication skills, with demonstrated ability to influence and conduct clinical medication reviews.
• Experience in leading quality improvement projects, clinical audits, and managing aspects of the Quality and Outcomes Framework and PCN Network Contract.
• Ability to plan, implement and evaluate pharmaceutical care programmes and to manage multidisciplinary teams.
