Job Summary

We have an exciting opportunity for Band 3 Healthcare Assistants who have a passion for admission prevention to join a core team to deliver a new and developing countywide Hospital at Home (H@H) service in Worcestershire. This is full time post, but we would consider applicants looking for part time. The post is based in the Single Point of Access (SPA) Worcester. The service will be countywide across Worcestershire so the ability travel across the county will be expected. Operational hours 8am-8pm seven days a week including Bank Holidays. Minimum 35.5 hours per week.

Responsibilities

  • Follow treatment plans to support the assessment and management of acutely unwell patients in the Community who otherwise might have been in an Acute Hospital setting with the support of a dedicated medical model for the Hospital @ Home.
  • Work independently, following treatment plans and actively communicating concerns/deterioration to clinical leadership as required.
  • Cooperate with stakeholders, particularly with Acute Trust colleagues.

Qualifications and Experience

  • Level 3 qualification (e.g. NVQ 3, A-level) in Care or Level 2 qualification in Care and expected to undertake Level 3 on appointment.
  • Will be expected to meet Care Certificate standards within 12 weeks of commencing employment.
  • Full UK driving licence.
  • Good IT skills.

Person Specification

  • Experience of working with older adults in a health/social care setting.
  • Desirable: Community experience of older adults with physical and/or mental health problems; experience of working in a rehabilitation or general medical setting; liaising with staff from different disciplines; medication administration.
  • Able to meet travel requirements with current full driving licence.
  • Willing to undertake further relevant training programmes.

Contract and Working Pattern

Contract: Permanent. Working pattern: Full-time, Part-time, Flexible working.

Salary

£24,937 to £26,598 a year pro rata for part time.
