PURPOSE OF THE JOB:
The post holder will be part of Adult Services, working to assess the social care needs of individuals in accordance with the wellbeing principles of the Care Act. They will adopt a personalised and strengths-based approach.
Responsibilities include promoting personalised support, facilitating service user choice and control, managing a caseload, conducting assessments with service users, carers, and multi-disciplinary partners, and identifying outcomes and asset-based goals to develop support plans. The post also involves assessing and managing risks, ensuring quality assurance, and safeguarding procedures are followed.
Knowledge of Adult Social Care
- Understanding of the Care Act, Mental Capacity Act, and DOLS.
- Knowledge of the London Multi-Agency Adult Safeguarding policy and procedures, and related legislation, including Making Safeguarding Personal.
- Awareness of best practices and initiatives across health and social care sectors.
- Understanding of the needs of Adults at Risk and those with long-term conditions and complex needs.
- Experience with assessment and support planning for diverse communities, focusing on strengths and independence.
Technical Skills
- Proficiency in IT tools such as databases, word processing, email, and spreadsheets.
Qualifications
- UK state-registered degree in Social Work, DipSW, or CQSW.
- Current Enhanced Criminal Records Bureau assessment.
