Hackney Council is looking for experienced Commissioning Reviews Social Workers to join the Adults Commissioning Reviews Team on a locum basis. This is a fantastic opportunity to play a key role in ensuring that commissioned services are delivering high-quality, cost-effective care while improving outcomes for adults across the borough.
Role
You will undertake person-centred reviews of adults receiving commissioned care, ensuring services continue to meet assessed needs, promote independence and provide value for money.
Key responsibilities
- Carrying out strengths-based reviews in line with the Care Act
- Ensuring placements are meeting quality, safeguarding and cost-effectiveness standards
- Reviewing and updating outcome-focused support plans
- Working closely with providers, internal stakeholders and partner agencies
- Supporting service development and identifying gaps in provision
- Contributing to commissioning activity, service improvement and market development
- Managing complex cases and supporting transitions where needs change
All reviews must be completed face to face – onsite visits are mandatory, however the team operates on a remote‑working basis with flexibility in day‑to‑day working. Monthly team meetings are held face to face.
Requirements
- Qualified Social Worker with Social Work England registration
- Minimum 2 years’ experience within a Commissioning Reviews Team
- Strong knowledge of the Care Act, Mental Capacity Act and Safeguarding
- Experience undertaking reviews and reassessments within adult services
- Ability to analyse service provision, improve outcomes and ensure cost efficiency
- Experience working collaboratively across multi‑agency settings
- Supervisory experience (supporting Reviewing Officers)
- Experience managing budgets and driving efficiencies
