Job Description

  • Ensure that the people of Gwynedd are at the heart of everything we do.
  • Assist vulnerable individuals to live their lives in the way they wish to live.
  • Ensure that individuals' needs are met in accordance with the Council's safeguarding arrangements and statutory responsibilities. There will also be an expectation to comply with all the requirements of the Care Act 2016.

Responsibility for functions

  • Staff - support for the registered Manager
  • The post-holder is expected to work as part of a team by supporting colleagues
  • Finance - supporting the team with efficient use of resources to contribute to effective budget management
  • Equipment - responsibility for any equipment used to carry out the job

Main duties

  • Support the individuals who use the service
  • Excel in building relationships with people to understand exactly what would make a difference to them in their lives and to empower the voice of the individual at every opportunity
  • Be innovative and creative when giving care in a way that fully includes people, that continuously strives to include people in even the simplest of activities and that enable or re-able people to take control of their own lives
  • Promoting the rights of the individual and encourage them to take positive and informed risks
  • Follow and develop staff to follow an ethos that emphasises the importance of working with individuals and promoting their independence
  • Collaborate with individuals and the team to support a range of services to meet with personal objectives
  • Contribute, develop, and monitor the care plan jointly with the individual, to promote and encourage him/her to live as full a life as possible by making use of any Resources and activities available in the community
  • Consideration will need to be given to the risks and the completion of the relevant documentation, including risk assessments
  • Contributing to the process of assessing, monitoring, planning and implementing activities in accordance with the needs of the service user
  • Working with people to assist and note barriers to prevent them from achieving their outcomes, and use information to discuss with the wider team, implement change and learn from experience
  • Be progressive and willing to make informed decisions independently, while also applying professional opinion and knowing when to attract support from the wider team and other professions

  • Develop and collaborate with the Home Management Team, to ensure compliance and working arrangements
  • Contribute to the process of assessing, monitoring, and implementing work practices in the home
  • Be responsible for maintaining all relevant administrative work in accordance with the guidelines set by the Service Support Manager on behalf of Cyngor Gwynedd
  • Assist with appointments and induction of new staff, ensuring appropriate DBS disclosures
  • Support any rota planning and arrangements in the home in the absence of the manager and the deputy manager
  • Operate and maintain IT systems
  • Ensure compliance with any risk assessment arrangements identified in risk assessments that are relevant to the work
  • Identify needs that cannot be met, record collaboration with the team to resolve these
  • Lead and respond to an emergency situation involving users and the building
  • Need to be qualified to undertake and monitor health tasks

Registration, training and supervision

  • Register and assist with the staff registration system and ensure that this is followed, in accordance with the requirements of Social Care Wales
  • Support the effective supervision of staff in accordance with the RISCA regulations including monitoring the workload of the member of staff
  • Annual Performance Review in accordance with the Council's policy
  • To practically supervise and demonstrate to staff
  • Support with staff training for them to get the necessary skills to undertake the work and to update their training programme
  • Maintain and support the procedure for administering and distributing medicines in accordance with the service policy and in collaboration with the local pharmacy and the Local Health Board
  • Be a support to staff
  • Be willing to undertake the role of service 'champion' and specialise in an area that will support the service, and a lead role that will develop other staff

Communication and relationships

  • Attend staff meetings and take a lead role when required
  • Work in partnership with colleagues in the health and care field, including social workers, community psychiatric nurses, occupational therapists and physiotherapists, GPs and 3rd sector partners
  • Attend multi-disciplinary reviews
  • Support and assist the process following complaints and compliments
  • Support with recording staff sickness absence arrangements in line with the service/Human Resources policies
  • Practice in a way which is in-line with the principles of the professional code of practice for social care
  • Give consideration to risk, safeguarding and the protection of adults
  • Deal with any concerns raised in relation to safeguarding adults promptly and appropriately by following the agreed procedure and informing the relevant professional officers without delay
  • Responsible for reporting any concern or suspicion that a child or vulnerable adult is being abused

  • Undertake any other reasonable duty which corresponds to the salary level and responsibility level of the job
  • Responsibility to report any concern or suspicion that a child or vulnerable adult is being abused

Special circumstances

  • Working unsociable hours occasionally as and when necessary
  • Expectation to occasionally work beyond the boundaries of the work area
  • The post-holder will be expected to be part of any arrangements that will be established on a rota basis and to work over weekends
  • There is an expectation that in an emergency you will be willing to move and work in a different service/field
