Overview
Are you a passionate & caring nurse with at least two years experience in community looking to progress and develop your clinical and leadership skills in a dynamic community nursing team? If so, we would like to meet you.
A vacancy has arisen within the Unplanned Care Team, which has an innovative approach to nursing care. It involves quick responses to patients in their home to assess, plan, manage and evaluate the presenting issue, prevent unnecessary hospital admissions, and help to facilitate early hospital discharges. You will support the clinical band-7 with the daily leadership and management of the team; and liaise with the wider multi-disciplinary team.
We are looking for a highly motivated applicant who is interested in meeting the challenge of working in the community with patients in their home setting. You will be working with a variety of disciplines within the multidisciplinary team to achieve the best outcomes for patients. You will be working on a rotational basis as part of a seven-day service covering 24-hours-a-day with the support of agile technology to help you deliver timely, high-quality care.
The successful candidate will have excellent clinical skills, demonstrate leadership qualities, implement high standards of patient care, and ensure a positive patient experience. Unplanned services are committed to supporting your continued professional development and we will support you in your ongoing career aspirations.
Senior staff nurses will have a minimum of 12-months community nursing experience. The postholder will assess, plan, implement, monitor, and review patient care. The senior staff nurse role supports the district nurse team leader with the daily service coordination and delivery; and is pivotal in the communication within the team.
To work as part of the community nursing team in unplanned care, ensuring high quality health care is delivered to patients in their own home. The post holder will be responsible for their own workload and work in collaboration with the wider Community Partnerships and Primary Care Networks. The post holder will be responsible for the assessment of health needs, delivery of care to the practice population, prevention of hospital admissions and facilitate early discharges from hospital. The unplanned service is delivered over a seven-day rota, covering a 24-hour shift pattern. Working agilely across the service.
