Overview
District Nurse Caseload Manager - Planned Care - City & South Locality
Responsibilities
Lead and develop a dedicated team of community nurses and healthcare support workers, ensuring safe, high-quality and compassionate care in people\\\'s homes and community settings.
Provide advanced assessment, care planning and case management, working in partnership with patients, families, carers, GPs, and wider multi-disciplinary teams.
Support prevention of hospital admissions, facilitate timely discharges, and enable people to remain independent for as long as possible.
Ensure team members are appropriately trained, supported and have the right knowledge, skills and competencies to meet role requirements.
Balance clinical and non-clinical duties (approximately 80% clinical and 20% non-clinical) and manage the day-to-day running of the caseload with clinical responsibility.
Qualifications / Experience
Qualified District Nurse with leadership abilities and experience in community nursing and planned care.
Strong assessment, care planning, and case management skills, with ability to mentor and develop staff.
Ability to work in partnership with patients, families, carers, GPs and wider multidisciplinary teams.
Requirements
Right to work in the UK; sponsorship eligibility as required for the role.
Registration with the appropriate regulatory body and compliance with professional standards.
Desirable: knowledge of safeguarding, discharge planning and community health care governance.
