We are recruiting a full-time Band 5 learning disabilities nurse to work in our health facilitation team. Based in the community, you will be part of a multidisciplinary team aiming to reduce health inequalities among adults with learning disabilities, supporting physical health, emotional wellbeing, and mental health.
The post holder will collaborate with adults with learning disabilities, their carers, professionals, and agencies to provide a high-quality, comprehensive, and dynamic specialist nursing service focused on maintaining and improving health and wellbeing. Responsibilities include:
- Working directly with service users with complex healthcare needs
- Supporting health facilitators
- Assisting health providers with care pathway adjustments
- Engaging in health promotion activities
You should be able to manage a caseload, prioritize demands, communicate effectively in accessible formats, and work independently. While we prefer applications from learning disabilities nurses, we will consider applications from nurses in other branches with relevant experience and a passion for reducing health inequalities.

Qualifications:
- Essential: First level registered nurse / NMC registration
- Desirable: First level learning disabilities nurse
Experience:
- Essential: Experience assessing healthcare needs of people with learning disabilities
- Desirable: Post-registration nursing experience supporting people with learning disabilities
