Are you an experienced care coordinator looking for a new challenge? Guild Care is seeking a Home Care Coordinator to join our compassionate, fun and professional team. As a coordinator you’ll play a key role in ensuring the smooth day-to-day running of our Home Care service. You will lead and support our team of Home Care Workers as well as our clients, helping them to live their amazing lives.

Responsibilities

  • Lead on the day-to-day management of the Home Care Team in collaboration with two other Care Coordinators, the Registered Manager, and two Field Care Supervisors.
  • Develop a detailed understanding of customers’ needs and wishes as described by the customers themselves.
  • Support, empower, coach, and encourage the team to understand what good care looks like.
  • Roster for customers, meet customers, and facilitate new care assessments, supporting the customer through their journey.
  • Collaborate with Field Care Supervisors to complete staff supervisions and appraisals, as well as database management.
  • Share responsibility for the Home Care service roster alongside two other Home Care Coordinators.
  • Be reliable, bring a sense of fun, and take ownership of all aspects of the role.

Working Arrangements

Based in the heart of Worthing, our Home Care service operates seven days a week, 365 days a year, delivering essential care and support to individuals in the community. We operate a seven-days-a-week schedule with on-call requirements; you will be required to work at least one day over the weekend every four weeks. Shifts are between 6.30am and 10.30pm. Salary is £12.21 per hour, with a minimum annual supplement of £2,539.

Requirements

  • Proven experience in a similar people coordinator role
  • Ability to lead a team with excellent communication skills
  • Ability to participate in an out-of-hours on-call rota
  • Experience within domiciliary care setting

Desirable Criteria

  • Experience of assessing the needs and risks of vulnerable adults
  • Experience of rostering staff and managing rotas
  • Experience of managing a team and completing staff supervisions and appraisals
