Brent Council is seeking a skilled Adult Social Worker to join its Hospital Discharge Team, supporting adults to move safely and confidently from hospital back into the community. This role is ideal for a practitioner who thrives in a fast-paced environment and is passionate about person-centred, outcome-focused practice.
