As a Healthcare Professional, you will provide high-quality care and support to individuals in need. This role involves assisting clients with daily living activities, personal care, and health-related support while maintaining effective communication ongoing with healthcare professionals, including nurses, doctors, and occupational therapists (OTs). You will play a crucial role in ensuring seamless collaboration between clients, families, and multidisciplinary teams to promote well-being and independence.

A healthcare professional's key duties include keeping the office informed about any ongoing visits from external healthcare professionals, such as district nurses (DNs), OTs, and GPs. You will liaise directly with the Registered Manager (RM) weekly, updating any CQC-related concerns, body mapping each client that we have area concerned post it open for, and body mapping new clients who are at high risk of pressure sores, as well as those who have been in hospital for longer than 48 hours.

Key Responsibilities:

  • Liaise with external healthcare professionals (DNs, OTs, GPs, and hospital teams) regarding client care.
  • Keep the office updated on ongoing visits from external professionals, ensuring accurate records are maintained.
  • Communicate weekly with the Registered Manager (RM) regarding:
    • CQC-related concerns or post-it notes requiring action.
    • Client updates and risk assessments.
  • Complete body mapping for:
    • All new high-risk clients (e.g., those at risk of pressure sores).
    • Clients with open post-it concerns.
    • Clients returning from hospital stays exceeding 48 hours.
  • Monitor and report changes in clients' conditions, escalating concerns to the RM and healthcare professionals.
  • Ensure accurate documentation of all client visits, healthcare interactions, and care plan adjustments.
  • Support carers with guidance and training on pressure area care and early identification of risks.
  • Adhere to CQC regulations and ensure compliance in all documentation and reporting.
