We have an exciting opportunity for an Advanced Clinical Practitioner to join our team and work in a well established practice, Gordon Street Medical Centre.

You will be supported by strong clinical leadership and a collaborative multidisciplinary team.

The hours for this role are 15 hours per week, to be worked on Wednesday and Friday between 08:00 and 18:30.

Main duties of the job

The Role

We are seeking a confident, autonomous Advanced Clinical Practitioner to work at Gordon Street Medical Centre.

You will play a key role in delivering safe, effective and patient centred care within a supportive multidisciplinary team.

Your responsibilities will include

  • Managing acute and routine presentations across both sites
  • Undertaking clinical assessments and diagnostics
  • Prescribing independently
  • Supporting continuity of care for patients with long term conditions
  • Working collaboratively with GPs, nurses and the wider MDT
  • Contributing to service development and quality improvement

  • Assess and holistically clinically examine patients with both minor and complex needs through history taking, physical examination and appropriate use of investigations to confirm diagnosis.
  • Plan, implement and evaluate an effective management plan for the patient, ensuring safe discharge/referral and/or transfer to an appropriate health care service.
  • Provide information and advice on prescribed or over-the-counter medication on medication regimens, side-effects and interactions.
  • Prioritise health problems and intervene appropriately to assist the patient in complex, urgent or emergency situations, including initiation of effective emergency care.
  • Refer and discharge safely ensuring that safety-netting advice is offered.
  • Triage telephone referrals, calls from external clinicians, patients and others into a range of services appropriately as required.

  • Utilise and demonstrate sensitive communication styles, to ensure patients are fully informed and consent to treatment.
  • Communicate effectively with patients and carers, recognising the need for alternative methods of communication to overcome different levels of understanding, cultural background and preferred ways of communicating.
  • Anticipate barriers to communication and take action to improve communication.
  • Maintain effective communication within the organisation and with external stakeholders.
  • Ensure awareness of sources of support and guidance and provide information in an acceptable format to all patients, recognising any difficulties and referring where appropriate.
  • Non-Medical Prescribing
  • If you are employed in a prescribing capacity, you must only prescribe within your scope of practice and comply with the gtd healthcare NMP governance registration processes.
  • You must maintain your knowledge and skills and prescribe within your clinical competencies.
  • The non-medical prescriber is required to comply with organisational NMP governance processes and adhere to local and national prescribing guidance.

  • Produce accurate, contemporaneous and complete records of patient consultation, consistent with legislation, policies and procedures.

Experience

  • Experience of working as senior clinician.
  • Accountability of own role and other roles in a practitioner-led service. Experience of leading/managing quality improvement.
  • Experience in leading/managing a team of nurses.
  • Experience of working in an unscheduled care service.
  • Experience of having worked in Primary Care.

V300 Non-Medical Prescriber (gateway dependent) dependent on professional eligibility.

  • Mentor/Teaching qualification. Clinical supervision training and experience.
