Clinical Prescribing Pharmacist - Hattersley Group Practice in Hyde

Job Summary

Gtd Healthcare is one of the largest NHS commissioned Primary and Urgent care providers in North West England. We have an exciting opportunity for an enthusiastic and dedicated Clinical Practice Based Prescribing Pharmacist to join our practice team and provide care for our patients at Hattersley Group Practice.

We serve around 5,400 patients in a diverse community experiencing significant socioeconomic deprivation and health inequalities. The Clinical Pharmacist will be part of a busy and proactive team working alongside clinical and non‑clinical colleagues to innovate and progress the clinical pharmacy service, enhancing patient care and outcomes.

The successful candidate will support the GP’s medicines optimisation to deliver change for the benefit of patients and/or the organisation.

Main duties of the job

• Provide support to other clinicians with regards to prescription and medication queries/repeat prescriptions, and demonstrate clinical reasoning and critical thinking, as well as undertaking comprehensive structured medication reviews and assessing/processing repeat prescription requests.
• Provide advice, care and medicines management for patients either via telephone/video or face to face.
• Act as a positive role model supporting junior staff while offering the ability to work flexibly between urgent care and primary care to provide safe, clinical and expert care.
• Undertake clinical medication reviews to proactively manage patients with complex polypharmacy.
• Ensure that services integrate with community and hospital pharmacy to help improve patient outcomes, improve access to healthcare and help manage workload.
• Lead on the development of innovation bids, new services or a new mode of service delivery to meet the demands of the changing healthcare environment.

• Experience of working within primary care and/or general practice as a clinical pharmacist, with an understanding of primary care prescribing, concepts of rational prescribing and strategies for improving prescribing.

• Able to obtain and analyse complex technical information and utilise data to improve clinical outcomes, including the review of medicine related incidents.
