gtd healthcare is one of the largest, not-for-profit, NHS commissioned primary and urgent care providers in North West England, operating 13 GP practices across the Greater Manchester area. We have a new and exciting opportunity for a Practice Nurse with a learning disability specialist interest to work across our primary care services to develop and enhance our current learning disability provision. The successful candidate will be an experienced and highly skilled Practice Nurse with extensive expertise in undertaking annual learning disability health checks for adults and young people. The role involves developing positive and collaborative partnerships to improve outcomes for people with learning disabilities across the Tameside, Manchester, and Oldham localities.
You will support gtd healthcare and its multidisciplinary teams to conform to the relevant mandatory, national and statutory legislation, frameworks and guidelines, and the wider team to shape and develop policies, guidance and standard operating procedures to support people with learning disabilities. Main duties of the job:
- Undertake annual health checks, including a physical examination when required and provide the patient with a health action plan.
- Support gtd healthcare and its workforce including Trainee Nursing Associates to conform to the relevant mandatory, national and statutory legislation, frameworks and guidelines.
- Improve the quality of care received by individuals with a learning disability across Primary Care, providing advice, guidance and support, responding to concerns from clinical staff in relation to patients with a learning disability.
- Develop effective and collaborative working across primary care including providing specialist nursing advice.
- Provide advice relating to assessments for people with a learning disability such as mental capacity assessment and best interest decision making under the Mental Capacity Act 2005.
- Assist with safeguarding inquiries, Learning Disability Mortality Reviews (LeDeR) and complaints involving people with learning disabilities.
- Ensure that all staff are aware and supported to assess if any reasonable adjustments are required for patients with learning disabilities.

Job responsibilities:
- To undertake annual health checks for all patients who have a learning disability across gtd primary care practices including a physical examination when required.
- To provide the service user with an annual health action plan.
- To support the service user to navigate services within primary care.
- To have an understanding of Quality Outcome Framework and use this to support annual health assessments.
- To act as a resource and point of contact providing advice, guidance and support for all gtd healthcare staff including trainee nursing associates when providing care for individuals with a learning disability.
- To facilitate the meeting of individual health care needs for people with learning disabilities, liaising with community learning disability teams to co-ordinate support for individuals with a learning disability who may require help.
- Raise the profile of the health care needs of people with a learning disability across care services to support improved communication.
- Contribute to the development of health care information and resources in accessible formats for both service users and their families / carers.
- Identify existing/potential barriers in accessing Primary Care services for people with a learning disability and promote initiatives to overcome these barriers.
- Support and enable gtd healthcare services to make reasonable adjustments within their care delivery for people with learning disabilities.
- Assist clinical areas in recognising potential barriers such as communication issues in relation to individuals with learning disabilities, and provide advice and support as required.
- To undertake benchmarking and audit against recognised national standards.
- Promote the early resolution of issues and complaints working closely with the appropriate Manager and Clinical Leads.
- Recognise and work within own competence and professional code of conduct as regulated by the NMC/HPC etc.
- Understand own role and scope and identify how this may develop over time.
- Produce accurate, contemporaneous and complete records of patient consultation, consistent with legislation, policies and procedures.

- Improve the quality of care received by individuals with a learning disability across Primary Care by providing advice, guidance, and support, and responding to concerns raised by clinical staff.
- Develop effective and collaborative working across primary care, providing specialist nursing advice related to the care of patients with learning disabilities.
- Provide advice and support relating to assessments for people with learning disabilities, including Mental Capacity Assessments and Best Interest decision-making in line with the Mental Capacity Act (2005).
- Assist with safeguarding enquiries, Learning Disability Mortality Reviews (LeDeR), and complaints involving people with learning disabilities.
- Work closely with the gtd healthcare Safeguarding Team and wider colleagues to develop, review, and update policies, guidance, and standard operating procedures that support people with learning disabilities.

- To be a highly competent and knowledgeable registered health professional with experience in supporting people with a learning disability within primary care.

Education and Development:
- To work in partnership with multi-professional/education staff to provide a comprehensive, innovative clinical skills and competency education framework.
- To work with the Head of Nursing, clinical leads, gtd academy and safeguarding leads, to develop a training needs analysis relating to learning disability requirements and development of teaching materials/training packages.
- To co-ordinate and assist in the delivery of training to meet identified needs specifically to ensure that colleagues are aware of and able to meet the needs of people with a learning disability.
- To contribute or deliver educational sessions to staff groups, liaising with other agencies and service users in order to gain their participation in the education delivery.
- To act as a resource for staff in training posts, including Student Nurses and Trainee Nursing Associates, to support the development of learning disability awareness and competence across gtd healthcare.

Non-Medical Prescribing:
