Diolch am eich diddordeb yn y swydd yma gyda Canllaw. Mae'r swydd yma gyda'r tîm Gofal a Thrwsio Gwynedd a Môn. Mae Canllaw yn is-gwmni i Grŵp Cynefin. Nod 'Canllaw' yw "galluogi pobl i fyw yn eu cymunedau". Rydym yn fenter gymdeithasol sy'n gweithredu ar draws Gwynedd ac Ynys Môn, gan ddarparu gwasanaethau tai mewn ymateb i anghenion pobl hŷn neu bobl fregus lleol.

Mae ein gwasanaethau yn cynnwys:

  • Canllaw Addasu - Gwasanaeth masnachol o gynnal addasiadau priodol yng nghartrefi pobl hŷn neu bobl fregus, er mwyn iddynt gynnal eu hannibyniaeth.
  • Canllaw Technegol - Gwasanaeth proffesiynol o ddylunio, cynllunio, archwilio a goruchwylio gwaith adeiladu, boed gwaith mawr neu fach. Yn gwasanaethu cleientiaid sy'n bobl hŷn neu bobl fregus.
  • 'Gofal a Thrwsio Gwynedd a Môn' – Gwasanaeth rhad ac am ddim i bobl 60 oed a drosodd i'w cynghori a chefnogi i gynnal, gwella neu addasu eu cartrefi.

Mae Grŵp Cynefin wedi bod yn darparu tai o ansawdd, sy'n ddiogel a fforddiadwy dros chwe sir gogledd Cymru a gogledd Powys i dros 8,000 o bobl ers 2014. Rydym yn gweithio'n galed i ddarparu tai a gwasanaethau rhagorol, i gyfrannu i ddatblygiad cymunedau cynaliadwy a gwarchod a hybu'r iaith Gymraeg gyda balchder.

Gwella llif cleifion drwy ysbytai a gostwng nifer o achosion sy'n gorfod mynd yn ôl i'r ysbyty. Darparu cyngor a chefnogaeth ar gyfer pobl hŷn a phobl anabl i drwsio, addasu a chynnal eu cartrefi, gan fod yn sensitif i'w anghenion a'u dymuniadau, i'w grymuso i fyw yn annibynnol a diogel yn eu cartrefi.

Gweithio tuag at ddarpariaeth lwyddiannus o wasanaeth Gofal a Thrwsio yng Ngwynedd a Môn. Mae'r swydd yn bartneriaeth rhwng Care & Repair Cymru, Canllaw a BCUHB ac yn weithredol yn bennaf yn Ysbyty Gwynedd ond hefyd yn gallu rhoi cymorth i gleifion sydd mewn ysbytai llai ar draws Gwynedd a Môn.
