Grovelands Medical Centre are looking for a Clinical Pharmacist with General Practice experience to work closely with our practice teams - to deliver our pharmacy vision, delivering excellent pharmacy-led services for patients registered with the practice. Experience in General Practice is essential.
Main duties of the job include:
- Up-to-date clinical knowledge and a thorough understanding of primary care prescribing.
- Medication reviews and experience of designing and implementing strategies for improving prescribing.
- Upholding the NHS commitment to addressing health inequalities and ensuring care is available to people from all communities.
- The ability to recognise personal limitations and boundaries, understand those of colleagues, and refer to other colleagues when necessary.
- Thriving in a devolved, flexible, autonomous environment.
About us:
- Deal with the repeat prescription system, acute prescription requests, management of medicines on transfer of care: discharge letters, treatment advice notes and medicines reconciliation.
- Provide telephone clinics for patients with questions, queries, and concerns about their medicines.
- Perform structured medication reviews and manage some aspects of the quality and outcomes framework and enhanced services.
- Work with the practice to develop and apply systems for safer prescribing.
- Develop good relationships between local community pharmacies and the PCN pharmacy team.
- Involvement with periodic meetings for identifying and reviewing prescribing and patient safety issues.
- Involvement in the Prescribing Quality Scheme, PINCER and QOF aspects for prescribing and audits.
- Work with care home staff to improve safety of medicines ordering and administration and conduct periodic care home visits.
- Work efficiently with other members of the pharmacy team, clinicians, and the admin team.
The role is pivotal to improving the quality of care and operational efficiencies, requiring motivation and passion to deliver excellent service within general practice. Experience within General Practice is essential. Independent Prescribing Qualification is desirable.
Job responsibilities include:
- Practicing at a senior level with excellent written and verbal communication skills.
- Experience of influencing others and managing face-to-face and remote clinical caseloads in primary care.
- Undertaking a patient-facing role in managing long-term conditions; working directly with patients to assess and treat conditions, as well as promote self-care.
- Developing and delivering medicines-related training for GP and ACP prescribers in the PCN, and education events for clinicians and patients.
- Leading and owning the Prescribing Quality Scheme, PINCER and QoF aspects for prescribing and audits.

- Qualifications: GCSE grade A to C.
