Rehabilitation Case Manager

Permanent | Full time and part time hours available

Location: Remote with occasional travel

Salary: up to £45,000 per annum (based on experience)

Are you a clinician with expertise in Brain Injury, Spinal Cord Injury, Amputation, Complex Orthopaedic, or Burn Injuries?

Our client is seeking an experienced and registered clinician to become part of their team on a permanent basis.

Main duties and responsibilities:

  • Conduct immediate needs assessments and proactively case manage clients with life-changing injuries.
  • Work collaboratively to identify and achieve client goals, providing tailored, goal-focused rehabilitation plans.
  • Use your excellent communication and clinical skills to empower clients to reach their full potential.

Qualifications & Experience:

  • HCPC, NMC, or equivalent professional registration.
  • A minimum of 2 years post-graduate clinical experience.
  • Full, clean UK driving licence.
