This is a role with responsibilities of a GP Assistant (GPA) to support the effective functioning of General Practice within the Primary Care Network (PCN). The post holder will work under the supervision of GPs and the PCN team to assist with clinical and administrative tasks, as well as to proactively support patients particularly those who are vulnerable or have complex needs to navigate health and care services and improve their health outcomes.

Main duties of the job:

  • Prepare patients for basic examinations in line with preventative screening and LTC monitoring (e.g., measuring BP, pulse, height, weight).
  • Perform simple clinical tasks following appropriate training (chaperoning).
  • Update patient records and ensure coding accuracy using clinical systems (e.g., EMIS).
  • Manage recall systems for chronic disease reviews, vaccinations and follow-up appointments.
  • Act as a single point of contact for patients identified for proactive care coordination.
  • Work closely with GPs, Social Prescribers, Pharmacists, Nurses, and other health professionals to ensure a joined-up care approach.
  • Help patients to understand and manage their conditions and support them to set goals and take control of their health.
  • Coordinate multidisciplinary team (MDT) meetings and ensure actions are followed up.
  • Maintain and monitor personalised care and support plans.
  • Identify and address gaps in care, ensuring patients attend reviews, screenings, and appointments.
  • Support the delivery of national PCN DES specifications such as Enhanced Health in Care Homes, anticipatory care, and personalised care.

Person Specification:

  • Experience working in health or social care
  • Working with clinical systems (e.g. EMIS)
  • Experience in a general practice setting
  • Multidisciplinary team working

Knowledge and Skills:

  • Strong organisational and communication skills
  • IT literacy
  • Ability to manage sensitive information confidentially
  • Good written and verbal communication
  • Motivational interviewing
  • Knowledge of long-term conditions

Qualifications:

  • Care Navigation or Health Coaching qualification
