SEL Integrated Care Board (NHS) has developed an initiative called Vital 5 which focuses on five key aspects of health, providing an overall picture of how healthy a person is and any areas they may wish to improve.

Responsibilities
- Conduct Vital 5 assessments with residents identified by GP practices or through community outreach.
- Offer personalised, one‑to‑one coaching to residents—prioritising those with high blood pressure—to help them set meaningful goals and overcome barriers to better health and wellbeing.
- Provide ongoing support for four to six coaching sessions, which may include follow‑up phone calls or attendance at appointments, tailored to each resident’s needs.
- Empower residents to take control of their own health and wellbeing by ensuring they have access to appropriate support, are aware of relevant services, and have a clear, person‑centred exit strategy in place prior to discharge.
- Maintain accurate and timely records of coaching sessions and outcomes using agreed systems and tools.
- Actively share key themes and insights to support the continued development of integrated neighbourhood working in Greenwich.
- Support the maintenance and updating of the database of local services and activities in Greenwich.
- Travel across Riverview PCN and work at various locations within the PCN to support residents.

Greenwich PCN Alliance Limited has been running since 2020 and consists of four PCNs: Blackheath and Charlton PCN, Eltham PCN, Heritage PCN and Unity PCN. The aim is to support the improvement of primary care across Greenwich by providing support to Primary Care Networks (PCNs) across Greenwich and recruiting additional roles via the Additional Roles Reimbursement Scheme (ARRS).
