Job Summary
The post holder will be employed by Greenwich Health, the GP Federation in Greenwich. Greenwich has seven primary care networks (PCNs) with list sizes ranging between 30,000 and 80,000 patients. The post holder will work for Riverview Health PCN, a group of five practices.
Due to changes in the ARRS Scheme to introduce General Practitioners, the practice seeks to employ a newly qualified General Practitioner (you must be within two years of certificate of completion of training at the start of employment) to work alongside our practice and PCN staff.
The GP will act as an autonomous practitioner, working independently and with other health‑care professionals to assess, diagnose and treat conditions of patients attending within a primary care setting. He/she will provide expert professional advice to patients, carers and colleagues and ensure the maintenance of clinical excellence. He/she will be professionally accountable and responsible for all elements of clinical practice in accordance with the GMC code of conduct, ensuring patient safety and quality of care.
The Role May Also Involve
Involvement in the provision of education and training to GP registrars and other staff.
Work in PCN‑wide schemes/services including some extended hours and some potential hub work.
Responsibilities
Make yourself available to undertake a variety of duties including surgery and online consultations, telephone consultations and queries, visiting patients at home, checking and signing/authorising repeat prescriptions, and dealing with queries, paperwork and correspondence in a timely manner.
Screen patients for disease risk factors and early signs of illness.
In consultation with patients and in line with current practice disease‑management protocols, develop care plans for health.
Provide counselling and health education.
Admit or discharge patients to and from the caseload and refer to other care providers as appropriate.
Record clear and contemporaneous consultation notes to agreed standards.
Compile and issue computer‑generated acute and repeat prescriptions.
Prescribe in accordance with the practice prescribing formulary (or generically) whenever clinically appropriate.
Visit patients during surgery time in an emergency if clinically indicated.
Undertake a fair share of on‑call sessions per week as allocated on the practice rota and participate in the extended hours enhanced service.
Undertake a fair share of home‑visiting sessions per week as allocated.
