Job Summary This is the perfect opportunity to be part of a diverse team providing assessment, advice, and referrals.As a Bank Home Visiting HCA you will be a key individual in executing Greenwich Health\’s home visiting service on behalf of participating practices. The service will not function efficiently without you. You will work independently at the patient\’s residence, in collaboration with clinical and logistical support, and you will receive induction support by shadowing a senior clinician for your first sessions to ensure comfort in the role.On your shift you will work independently, using your own initiative. You will perform foot checks, phlebotomy, height and weight measurements, peak flow and a range of other tasks for housebound patients. The service operates 1-2 days a week, with shifts available accordingly. Security and safeguarding measures are in place, including a GPS tracking alarm and regular communication with a coordinator. A lone worker policy is in place.
Main duties and responsibilities HCA Home Visit / Long Term Conditions Protocol: Work with the admin coordinator, sending a PEM to the patient\’s practice and highlighting any areas that need referrals.Check equipment and paperwork required for the home visit; ensure all is returned to base and equipment used is replaced.Complete the checklist and sign-in/sign-out sheet.Carry out the required patient checks and record all findings onto the patient\’s EMIS record.Ensure communication with the service phone before and after each home visit.Log start and end mileage in the mileage log.Return to the office and replace any used equipment.Gather observations and information as an HCA, including: examination, height, weight, BMI, BP, pulse, pulse rhythm, oxygen saturation, foot check (diabetes), peak flow (asthma), lifestyle factors (alcohol, smoking, exercise), and relevant bloods when applicable.
