As a Lifestyle Co-ordinator, you will:
- Develop, deliver and regularly review individual lifestyle plans with each resident, based on their life story, strengths, interests, culture, faith, communication needs, preferences, aspirations and current abilities.
- Work with residents, families, advocates and colleagues, with appropriate consent, to understand what matters to each person and reflect this in care planning.
- Coordinate an inclusive weekly and longer-term lifestyle programme that is responsive to residents’ wishes and changing needs, including evenings, weekends and seasonal or culturally significant events where agreed.
- Complete risk assessments for activities, outings, equipment and visiting providers.
- Build constructive links with relatives, volunteers, schools, faith groups, entertainers, local organisations and community services.
- Manage the agreed lifestyle budget and resources responsibly, keeping accurate financial records and ensuring equipment is safe, suitable, accessible and maintained.
- Experience of planning or delivering meaningful activities, wellbeing support, hospitality, community engagement or care services (Essential)
- Understanding of person-centred practice, dignity, equality, safeguarding, consent, confidentiality and professional boundaries (Essential)
- Ability to work flexibly, including occasional evenings, weekends, events and outings where required (Essential)
- Relevant qualification in health and social care, activity provision, wellbeing, dementia care or a related field (Desirable)
- Previous experience in a care home or supporting older people (Desirable)
