To provide safe and effective clinical triage for patients contacting or attending the GP practice with urgent or same‑day healthcare needs. The post holder will assess patient symptoms, prioritise care appropriately, provide clinical advice, and direct patients to the most suitable clinician or service.
Main duties of the job
- Clinical Triage
- Undertake telephone triage assessments for patients presenting with acute health concerns.
- Assess symptoms and identify red flags or deteriorating conditions.
- Prioritise patients according to clinical urgency.
- Provide evidence‑based advice and self‑care guidance where appropriate.
- Escalate urgent or emergency cases promptly to GPs, emergency services, or senior clinicians.
- Book patients into appropriate clinics or refer to alternative services including UTC, community pharmacy, or emergency care.
- Clinical Care
- Deliver nursing interventions within scope of practice.
- Support same‑day urgent care clinics where required.
- Perform clinical observations and basic procedures as appropriate.
- Administer medication inline with practice protocols and NMC standards.
- Communicate clearly and compassionately with patients and carers.
- Provide reassurance and health education.
- Maintain accurate and contemporaneous electronic patient records.
- Team Working
- Work collaboratively with GPs, ANPs, pharmacists, HCAs, reception staff, and multidisciplinary teams.
- Participate in daily clinical huddles and handovers.
- Support safe patient flow within the practice.

Greengates Medical Group is a single practice PCN with 5 sites under our umbrella, 3 in Beverley and 2 in Cottingham covering over 30k patients. We have 10 partners, 3 salaried GPs, ACPs, a Practice Pharmacist and qualified Medicines Management Team along with a fully skilled care home team including DOT Lead and a cohort of Paramedics. The above are all supported by a strong management team, Practice Nurses, MSK FCPs and a reliable administrative support team.
Job responsibilities
- Same‑day appointment triage.

- Signposting patients to appropriate services.
- Supporting urgent GP clinics.
- Safety‑netting and follow‑up advice.
Qualifications
- Registered Nurse with current NMC registration.
- Strong communication and decision‑making skills.
- Ability to work under pressure and prioritise workload.
- Competence in clinical assessment and recognising deterioration.
- IT literacy and accurate record keeping.
- Experience in general practice, urgent care, or community nursing.
- Knowledge of primary care pathways.
- Familiarity with SystmOne.
- Minor illness experience.
