Green Valleys Health is lookingfor a friendly, well organised and dedicated Pharmacy Technician to become anintegral member of the general practice team, working within their professionalboundaries in practices within the Primary Care Network (PCN).
This role willinvolve working under the supervision of the GP prescribing lead and alongsidethe existing Pharmacy Technician and Prescription Clerks within theprescription team to ensure safe, accurate and timely supply of prescribedmedicines to patients.
The Pharmacy Technician will provide technical andadministrative support to the GP prescribing lead, other clinicians, and thewider practice staff.
They will support the PCN and practice clinical teamswith queries around medication from patients.
They will be an advocate formedication safety, reduced medicines wastage and medication optimisation acrossthe PCN and practices.
Main duties of the job
Assistthe Prescription Clerks with medicines reconciliation to ensure accuratetransfer of medications across health settings when receiving dischargesummaries and clinic letters from secondary care.
Conduct medicines optimisation tasks including effective medicinescounselling, supporting medication reviews and medicines reconciliation.
Maintain strong working connections with local pharmacies toensure continued patient safety between settings and prompt query resolution.
Provide specialist expertise, where competent, to address both thepublic health and social care needs of patients including lifestyle advice, service information, and help in tackling local health inequalities.
Maintain a central role in the clinical aspects of shared careprotocols and local formulary and liaise with hospital specialists for morecomplex patients.
Support the implementation of national prescribing policies andguidance within GP practices, care homes and other primary care settings.
Undertake medication safety tasks such as high-risk medicationmonitoring and eclipse radar safety alerts.
Provide support and guidance to the prescription clerks throughsupervision and query resolution.
Work collaboratively with the existing Pharmacy Technician todevelop prescription team related policies and procedures and maximise patientoutcomes.

- Provide specialist expertise, where competent, to address both thepublic health and social care needs of patients including lifestyle advice, service information, and help in tackling local health inequalities.
- Maintain a central role in the clinical aspects of shared careprotocols and local formulary and liaise with hospital specialists for morecomplex patients.
- Assist in the delivery of medicines optimisation and managementincentive schemes and patient safety audits.
- Support the implementation of national prescribing policies andguidance within GP practices, care homes and other primary care settings.
Thiswill be achieved through undertaking clinical audits, supporting qualityimprovement measures and contributing to the Quality and Outcomes Framework(QOF) and enhanced services.
- Undertake medication safety tasks such as high-risk medicationmonitoring and eclipse radar safety alerts.
- Provide support and guidance to the prescription clerks throughsupervision and query resolution.
- Work collaboratively with the existing Pharmacy Technician todevelop prescription team related policies and procedures and maximise patientoutcomes.
- Attend a formal appraisal with your manager at least every 12months.
Once a performance/training objective has been set; progress will bereviewed on a regular basis so that new objectives can be agreed.
- There is potential scope for the Pharmacy Technician to engage in Pharmacy Technician-led medicines optimisationclinics following appropriate training and under the guidance of theprescribing lead GP.

Qualifications
- Registered with the General Pharmaceutical Council (GPh C)*
- *We are open to accepting students that are due to register as a Pharmacy Technician within the next 3 months if the role is right for them.
- ACT accredited
- Completed Primary Care Pharmacy Education Pathway (PCPEP) or equivalent
Experience
- Ability to work independently and collaboratively as part of a team
- Willingness to learn and advance within the role
- Good organisational skills
- Able to work within own professional and clinical competencies
- Evidence of CPD completion
- Experience working in primary care and community pharmacy and/or secondary care
- Understanding and knowledge of healthcare provision in GP practices, QOF and enhanced services
- Good clinical system IT knowledge of EMIS/Systm One/Vision/Docman/Eclipse
- Good knowledge of local formulary and guidelines (BNSSG), as well as national guidance (NICE, BNF etc)
- An appreciation of the new NHS landscape including the relationships between individual practices, PCNs and the commissioners
