The Care Coordinator is responsible for managing and coordinating care visits for clients with multiple, high-acuity needs. This role involves working collaboratively with healthcare and social care providers, clients, and families to ensure we deliver high quality person centred care. The Complex Care Coordinator ensures seamless communication among teams and optimises resource utilisation in order to ensure all our packages are covered.
Key Responsibilities- Complex Care Planning: Develop, implement, and monitor care visits for clients with high-acuity needs.
- Client Advocacy: Act as a liaison between clients, families, healthcare providers, and social care teams to ensure comprehensive support.
- Interdisciplinary Collaboration: Facilitate communication among medical professionals, therapists, social workers, and community organisations to provide holistic care.
- Crisis Intervention & Risk Management: Identify and mitigate risks, providing crisis intervention when necessary.
- Transition & Discharge Planning: Assist in planning and coordinating transitions between care settings, including hospital discharge, residential care, and home-based support.
- Resource Management: Identify and coordinate community resources, financial assistance, and social support programs to meet clients' long-term care needs.
- Documentation & Compliance: Maintain accurate, detailed, and up-to-date client records in compliance with social care regulations and policies.
- Quality Improvement: Participate in process improvement initiatives to enhance the effectiveness and efficiency of care services.
- A passion to deliver outstanding care.
- Experience in a senior role within the Health and Social Care Sector.
- Awareness of coordinating and managing care packages.
- Strong organisational, communication, problem-solving, and crisis management skills.
- An understanding of social care regulations, healthcare systems, chronic disease management, and best practices in client centred complex care.
- Ability to work independently and as part of a multidisciplinary team.
- Good IT skills preferably in Google.
- Must be a driver with access to a vehicle to support with care if necessary.
