Description:Part time Locum Therapist is required for 3 days per week in an INDEPENDENTLY OWNED mixed practice near Birmingham, West Midlands
• Monday, Tuesdays & Thursdays
• Hours 10am – 6pm
• £11.50 per UDA
• Established list
• 3 surgery practice
• Digital X-rays, OPG & Bellmont Chair
• Experienced nurse support
• Free Parking on site
• Available immediately, to run till end of September 25

- Full NHS Performer Number
- Local Area Team confirmation
- GDC certificate
- UK Blood test report showing Hep B immunisation and titre levels, Hep C, HIV and TB results dated within last 5 years
- Indemnity certificate

- Enhanced DBS (within the last 12-36 months)
- Clinical references covering places of employment over past 2 years (minimum 2 referees)
