This is an exciting opportunity for an experienced Mental Health Nurse, Social Worker or Occupational Therapist to join our specialist service for people experiencing a first episode of psychosis. We are recruiting two Band 6 Mental Health Practitioners / Care co-ordinators for teams covering the area of Manchester.
Main Duties & Responsibilities
- Act as Care-Coordinator for appropriate clients under the Care Programme Approach (CPA).
- Be a full member of the multi-disciplinary team (MDT).
- Effectively manage own caseload accepting referrals from agreed agencies in accordance with agreed operational procedures.
- Assess, care-plan, provide interventions and evaluate and record community mental health interventions delivered to clients on own caseload.
- Undertake formal mental health assessments of new clients within own caseload.
- Undertake assessments of clients as part of the EIT duty system in accordance with agreed operational procedures.
- Contribute to the service duty roster for their team locality.
- Manage discharges from the service and transfers to other services as appropriate for service users who are not accepted for the full First Episode Pathway service following assessment.
- Undertake risk assessment and risk management for individual service users and provide advice to other professionals on acute and chronic aspects of risk assessment and risk management.
- Work as part of our integrated MDT, attend daily meetings to feed back the outcomes of assessments undertaken, and participate in a clinical decision-making process to determine the most appropriate management plans for all service users who have received an assessment.
- Maintain a high standard of clinical record keeping, including electronic data entry, and ensure compliance with any associated performance standards.
- Liaise professionally and sensitively with other involved parties to obtain further information pertinent to the holistic assessment process.
- Provide advice, guidance and consultation to peer and junior professionals within the service, both individually, and by attendance at team referral and review meetings.
Teaching, training and supervision
- The post holder will receive regular clinical and managerial supervision.
- Remain informed of current research and guidance to support own professional work and contribute related knowledge to the MDT.
General
- Contribute to the development and articulation of best practice across the service, taking part in regular professional supervision and appraisal.
- Maintain up to date knowledge of legislation, national and local policies, and issues in relation to both the specific client group and mental health.

Person specification
- SWE/HCPC/Nursing Experience in an EIP Service.
- Minimum two years post qualifying experience of PANSS assessments.
