We are seeking an experienced Advanced Clinical Practitioner to join our newly formed Referral and Assessment Hub (RAH) in Manchester. The RAH is an essential part of Manchester's journey to a fully transformed model of community care. The hub provides swift triage and assessment of service users, adopting a system-wide approach to identifying an onward pathway.
Main duties of the job:
- The ACP will work as part of an MDT to provide senior leadership and advice to practitioners and referrers within the Hub.
- They will be experienced in holistic, trauma-informed assessment and formulation, identifying goals and working in a solution-focused way.
- The role will involve both direct and indirect working by seeing some service users in a clinic setting and providing advice and consultation to the wider team.
- The role will be supported by a Consultant Psychiatrist, Pharmacist, Mental Health Practitioners, administrators and staff from the VCSFE sector.
- As this is a new service, there is an expectation that the role will be key in developing the service to be safe, effective and innovative in the way it offers care.
