The predicted salary is between 36000 - 60000 Β£ per year.

The post holder will provide advanced and expert level of practice (including non-medical prescribing) within the adult Community Mental Health Team leading to improved patient outcomes. The Advanced Clinical Practitioner (ACP) will act as a professional resource for the multi-disciplinary team (MDT), patients and carers within the scope of their practice and competence. The ACP will understand professional accountability and legal frameworks for advanced practice and be professionally accountable and legally responsible for all aspects of their work. The ACP will demonstrate safe practice in line with Trust policies and procedures.

Main duties of the job include:

  • Supporting the clinical and operational team to deliver on service improvements in the quality of care and safety through service development, evaluation, and audit.
  • Being an integral part of the MDT supporting clinical decision making and service pathways, developing nurse-led clinics and providing brief interventions.
  • Holding a case load and leading assessment, formulation and prescribing of care planning including discharge planning within agreed protocols and procedures.
  • Demonstrating clinical and practice expertise via interventions, supervision and modelling good practice.
  • Contributing to a CMHT education and training programme across the community teams within Bolton.
  • Participating in the Bronze on site / Bronze on Call rota.

The core hours are Monday to Friday 9-5, although some flexibility, including weekend / BH working may be required.

Essential criteria for the role includes:

  • Registered first level nurse.
  • MSC in Advanced Clinical Practice.
  • Nurse prescribing qualification.
  • Experience of working with clients with complex needs.
  • Compiling protocols and clinical guidance.
  • Experience of working at a senior clinical nurse level.
  • Experience of completing audits.

Knowledge and skills required:

  • Advanced Clinical Practice skills.
  • Management of service users with complex needs.
  • Clinical examination skills.
  • Accountability of own role and other roles within the team.
  • Extensive knowledge of psychological, social and biological factors.
  • Clinical supervision training and experience.
  • Experience of coordinating & leading on audits, action plans & projects.
