Work in a Strengths Based way, maximising independence, and promoting
wellbeing. Assess complex mental health and social care needs of service
users and carers, and the provision in place aimed at meeting this need.
Assess and plan in line with the requirements of the Care Act (2014), s.117
of Mental Health Act 1983 (as amended by the Mental Health Act 2007),
Mental Capacity Act 2005.
Apply theoretical knowledge and understanding of psychology, sociology,
psychiatry and physical science to develop a case formulation describing an
individual’s strengths and challenges.
Produce a support plan for providers which meets the desired outcomes of
the individual but also maximises independence. Commission/implement
the right level of package of care at the right time so that independence is
promoted and resources maximised.
Prepare applications to funding panel following review.
Produce court reports for the Court of Protection.
Convene and attend meetings in accordance with s117 and CPA
requirements ensuring health and social care aftercare provisions and
legislative requirements under s117 are met.
If qualified AMHP- Act as an Approved Mental Health Professional (AMHP),
balancing the human rights of the service user within a legislative and
statutory framework alongside the issues of risk and safety to achieve the
least restrictive alternative.
Where applicable make referrals to Client Financial Services.
Actively access and evaluate ongoing up to date research to underpin,
develop and inform clinical practice.
Nurse Applicants-Confidently complete Nursing Needs assessments, CHC
screens and attend CHC MDTs. Utilise side effect monitoring tools eg
GASS and Lunsers
Develop an expertise in the bio psycho social assessment of
independent living ability and be able to select and use assessments
to illuminate underlying issues affecting independent living such as
skills, routines or volition
Advise the MDT and other agencies on the factors impacting on
independent living ability of the service user and support needed.
Make recommendation to improve the residential environment of the
service user in relation to occupational participation and the
achievement of their personal goals.
From assessment and formulation, advise the MDT, funding panel,
housing and care agencies, local authority and court of protection
regarding the ability of the service user to live safely and
independently and the support needed to do so.

Adhere to the principles of the Care Act in relation to the safeguarding of
adults with Care and Support needs. Raising referrals and contributing to
multi agency safeguarding planning meetings. Work in an empowering and

plan enabling desired outcomes to be achieved.
Work in line with Manchester Safeguarding Partnership and GMMH
safeguarding policies.
Identify safeguarding concerns for children and young people and liaise with
Children’s Social Care, Early Help to ensure information is shared and
appropriate action taken.

The Review Team is a multi disciplinary with team members from backgrounds that incorporate Social work, registered mental health Nursing and Occupational therapy, who review individuals who have a funded package of care. Where appropriate the team will look at lesser supportive packages to enable increased independence.
Completing reviews of packages of care funded by Manchester City Council and Manchester Integrated Board.
Facilitating move on from funded placements where appropriate.
Working in Strength based way, so independence is promoted, and resources maximised.
Working in accordance with Key legislation e.g. Care Act (2014), Mental Health Act (1983), Mental Capacity Act (2005) and Heman Right Act (1998), Saturday (midnight to midnight) and any week day after 8pm and before 6am
