We are seeking a highly skilled and motivated ANP to join our dynamic team at a busy GP surgery in Great Oakley for a minimum of 4 sessions. This is an exciting opportunity for an experienced nurse to play a pivotal role in delivering high quality patient care within a supportive, multidisciplinary environment. We are rated good at our last CQC inspection. We are a Systmone practice with strong QOF outcomes.
Main duties of the job
- Provide autonomous, evidence based clinical assessment, diagnosis, treatment and management for a wide range of acute and chronic conditions.
- Conduct physical examinations, order and interpret diagnostic tests and prescribe medications within scope of practice.
- Work collaboratively with GP's, practice nurses, healthcare assistants, and admin team to ensure seamless patient care.
- Develop and implement care plans tailored to individual patient needs.
- Lead health promotion and disease prevention initiatives with the practice.
- Contribute to clinical audits, service development and quality improvement activities.
- Provide expert advice, education and support to patients and their families.
- Should be comfortable doing triage.

We have around 15,600 population with a team of 8 GP's, 2 ANPs, 1 Pharmacist, 4 nurses, 1 Trainee Nurse Associate, Physiotherapist, Care Coordinators, Social Prescriber and Paramedic.

Job responsibilities
- Responsible for attending to patients who wish to be seen by a health care professional; telephone triage diagnosing and treating patients presenting with minor illness seeing and advising people in respect of their continuing medical and nursing needs.
- These patients will have booked appointments at fifteen-minute intervals. There is a requirement to see extra or emergency patients.
- The nurse practitioner will need to prioritise and triage the needs of patients accordingly making any necessary referrals for investigations in the appropriate manner.
- Patients with problems needing referral to secondary care should be discussed with the registered GP before making such referral.
Key Responsibilities
- Assess and triage patients, including same day triage, and as appropriate provide definitive treatment (including prescribing medications following policy, patient group directives, NICE (National) and local clinical guidelines and local care pathways) or make necessary referrals to other members of the primary care team.
- Manage undifferentiated undiagnosed condition and identify red flags and underlying serious pathology and take appropriate actions.
- Use complex decision making to inform the diagnosis, investigation, complete management of episodes of care within a broad scope of practice.
- Actively take a personalised care approach and population centred care approach to enable shared decision making with the presenting person.
- Complete the relevant training in order to provide multi-professional clinical practice and CPD supervision to other roles within primary care, for example first contact practitioners and the personalised care roles.
- Provide general and specific health screenings to the Practice patients (within agreed protocols) with referral to other members of the primary and secondary health care team as necessary.
- Perform investigatory procedures requested by the GPs and yourself.
- It will be necessary to attend and contribute to various Practice meetings as requested. The only reason for not attending will be annual, study or sick leave.
Administration and professional responsibilities
- Participate in the administrative and professional responsibilities of the Practice team.
- Ensure accurate and legible notes of all consultations and treatments are recorded in the patients notes.
- Ensure the clinical computer system is kept up to date with accurate details recorded and amended.
- Ensure accurate completion of all necessary documentation associated with patient health care and registration with the Practice.
- Ensure collection and maintenance of statistical information required for regular and ad hoc reports and audit.
- Attend and participate in Practice meetings as required.
- Restocking and maintenance of clinical areas and consulting rooms.
Training and personal development
- Training requirements will be monitored by yearly appraisal and will be in accordance with Practice requirements.
- Personal development will be encouraged and supported by the Practice.
- It is the individual's responsibility to remain up to date with recent developments.
- Participate in the education and training of students of all disciplines and the introduction of all members of the Practice staff where appropriate.
- Develop and maintain an in house nurse-training programme reporting to the Nurse Manager.
- Maintain continued education by attendance at courses and study days as deemed useful or necessary for professional development ensuring PREP requirements are met.
- If it is necessary to expand the role to include additional responsibilities, full training will be given.
- Develop and maintain a Personal Learning Plan.

Person Specification
Experience
- Proven ability to work independently and as part of a multidisciplinary team.
- Excellent clinical assessment, diagnostic and prescribing skills.
- Strong communication, organisational and leadership abilities.
- Clinical experience in primary care or community setting preferred.
Qualifications
- Registered Nurse with NMC.
- Postgraduate qualification in Advance Nursing Practice or equivalent.
- Must be able to work Monday or Friday full day.
- Able to Triage.
Knowledge and skills
- Ability to provide quality care.
- Good interpersonal and communication skills.
- Experience of Systm One.
