We are a training practice that is innovative and forward thinking, providing high quality care whilst maintaining traditional values of patient centred family orientated medicine. Main duties of the job:

  • Chronic disease management
  • Adult vaccinations & injections: including GnRH & Denosumab
  • Cervical screening
  • Contraceptive services: Depo-Provera, pill initiation and review
  • FeNO

About us:

We are a well established GMS practice in a market town on the edge of the Ribble Valley. Impressive spacious modern premises with wide array of clinical facilities and multi-disciplinary team onsite. We have 9400 patients cared for by a team of 6 Partners, 1 Salaried GP, 1 Advanced Practitioner, 1 Trainee Advanced Practitioner, 2 Practice Nurses, 1 HCA and a large Reception and Admin team. We also have our PCN team consisting of Pharmacists and Pharmacy Technicians, First Contact Physio, GP Assistant, Mental Health Team, Health & Wellbeing Coach and a weekly ante-natal session led by the Midwife. We have excellent QOF attainment and are rated GOOD by CQC. EMIS is our clinical system. We are an accredited training practice taking medical students, FY2 and STs.

Job responsibilities:

To provide high-quality nursing care to patients within the GP practice, promoting health, preventing illness, and supporting the effective management of chronic diseases in accordance with practice policies, NICE guidelines, and professional standards. Key responsibilities and clinical duties include:

  • Deliver evidence-based nursing care to patients within the practice setting.
  • Undertake routine nursing procedures including:
    • Cervical cytology
    • Childhood and adult immunisations
    • Travel vaccinations (where appropriate)
    • ECGs
    • Venepuncture
    • Blood pressure monitoring
    • Health checks
  • Support the management of long-term conditions such as:
    • Diabetes
    • Asthma
    • COPD
    • Hypertension
  • Provide lifestyle advice and health promotion to patients.
  • Administer medications and vaccinations in line with Patient Group Directions (PGDs) and practice protocols.
  • Ensure accurate and contemporaneous patient records are maintained using the clinical system.
  • Participate in infection prevention and control procedures.
  • Provide holistic, patient-centred care.
  • Maintain patient confidentiality at all times.
  • Communicate effectively with patients, carers, GPs, and the wider multidisciplinary team.
  • Support safeguarding responsibilities for children and vulnerable adults.
  • Maintain accurate clinical documentation.
  • Participate in clinical audits and quality improvement initiatives.
  • Monitor and maintain stock levels and clinical supplies.
  • Ensure equipment is maintained and used safely.
  • Maintain current NMC registration.
  • Participate in mandatory training and continuing professional development (CPD).
  • Work within the NMC Code of Conduct and scope of professional practice.
  • Participate in practice meetings and training sessions.
  • Contribute to the achievement of QOF and enhanced service targets where appropriate.

Qualifications:

  • Registered General Nurse (RGN) with current NMC registration.
  • Chronic disease management qualifications or experience.
  • Cervical cytology qualification.
  • Ability to work independently and as part of a team.
  • Competent IT skills and experience using clinical systems.
  • Experience in vaccinations, immunisations and injections.
  • Experience working within primary care or general practice.
  • Diabetes Diploma.
  • COPD Diploma.
  • Experience with EMIS.
