The predicted salary is between 32000 - 48000 £ per year.

Office hours at home then travel for face to face assessments in the North West, with occasional national coverage. Monday to Friday 9am to 5pm.

Key Responsibilities:

  • Completing desk-based and face to face assessments of referred people for support.
  • Ensuring an APIE approach - Assessing, planning, implementing and evaluating.
  • Reviewing reports and evidence outcome measures to inform clinical practice, business performance and quality measures.
  • Maintaining continuous professional development and being able to produce your CPD folder upon request.
  • Identifying and mitigating complex risks for a community setting.
  • Reviewing pre-admission assessments in a timely and professional manner.
  • Producing clinically informed assessments with robust risk assessment and identifying risk mitigation measures.
  • Identifying measures to ensure people can be enabled through positive support.
  • Attending internal and external MDT meetings to further inform the assessment and risk mitigation process.
  • Effective time management.
  • Record keeping, ensuring that accurate, safe and contemporaneous records are maintained.
  • Effectively working alongside stakeholders and other external professionals.
  • Proactively nurturing relationships with key stakeholders advocating and promoting the Gray Healthcare model and the mutual benefits of co-working.
  • Ensuring adherence to organisational values, policies, and ethical standards across all operations.

Essential Skills and Experience:

  • Registered RMN or RNLD.
  • Experience of assessing and working with people with a variety of complex needs around their mental health, learning disability, ABI, personality disorder, autism and forensic backgrounds.
  • Understanding of continuing healthcare or transition of individuals moving from hospital/residential settings into the community and a collaborative, solutions-focussed approach to any issues associated with this.
  • Applied knowledge of the Mental Health Act, CTOs and the Mental Capacity Act, Deprivation of Liberty Safeguards and Best Interest Decision making.
  • An expert in assessment and management of risk, innovative thinker and natural communicator with strong report writing skills and computer literacy.
  • Willingness to travel to conduct face to face assessments in an agreed geographical area.
  • Full UK Driving Licence and access to a vehicle.
