About this job:
- Operate as part of a multidisciplinary team providing Crisis Intervention and Intensive Home Treatment to CYP countywide.
- Use their initiative and take responsibility for themselves and the quality of their work and the service they provide to CYP.
- Proactively engage in formal and informal education of Acute Trust staff and other partnership agencies, families and children and young people.
- Actively and creatively engage CYP and carers encouraging them to make appropriate use of the mental health, social care and voluntary services available as part of a process of recovery and crisis management.
- Undertake full psychosocial assessments with those in need of such.
- Ensuring thorough, precise and timely documentation and dissemination of information in line with Trust policy and procedure.
- Undertake a supervisory role to junior staff in line with trust policy and procedure.
- Undertake acute risk assessment and safety planning for extremely vulnerable client groups during times of crisis.
- Deliver high quality, evidence based care to a defined team caseload.
